  • Completed DS-11: Application for a U.S. Passport
  • Be sure to complete all questions and provide your social security number
  • Two color photos, where you are not wearing a hat or glasses
  • Proof of U.S citizenship, such as a certified birth certificate or expired passport
  • Proof of identity, such as a driver’s license or state ID
  • Proof of travel that shows the date you plan to leave

If your name has changed and does not match your citizenship or identity documents, you will also need to provide proof of your legal name change. If you are using an expeditor, you will also need to provide an authorization form or letter.

The requirements for a U.S. passport renewal is the same as routine service. 

  • You need to complete the DS-82: U.S. Passport Renewal Application for Eligible Individuals. 
  • You need to include your social security number and complete all pages of the form. 
  • You must submit your current passport with the application.
  • It should have been issued within the last 15 years, issued when you were at least 16 years old, and not have any damage. 
  • This passport will be returned to you. 
  • You must also include two photos and the application fee. 
  • The photos must be in color and you cannot be wearing eyeglasses or a hat. 
  • If you are changing your name on the passport, you must also submit the appropriate documents.

The speed with which you can get a U.S. passport largely depends on when you need to travel. 

  • You can get a U.S. passport in one day if you need life-or-death emergency passport service. 
  • Those who have urgent travel plans that require them to travel outside the United States in less than three weeks can apply for expedited passport service. 
  • You should apply for the passport within two weeks of your travel date. 
  • If your travel date is in less than eight weeks for any reason, you can apply for expedited service. 
  • Those whose travel outside the U.S. is more than eight weeks away can use routine service, which typically takes six to eight weeks to receive the passport.

A life-or-death emergency is defined as death, injury, or serious illness in your immediate family, which requires you to travel outside the U.S. within three business days.
